Turf Toe
Turf Toe
What Is Turf Toe?
Turf toe is a sprain of the ligaments surrounding the big toe joint, most commonly occurring when the toe is forcefully bent upward. It often results from athletic activity on hard surfaces such as artificial turf—hence the name. At SoCal Foot and Ankle Doctors, with locations in Santa Monica and Cedars-Sinai Medical Towers, we diagnose and treat turf toe with a range of conservative and advanced therapies tailored to each patient’s activity level and recovery goals.

What Causes Turf Toe?
Turf toe typically occurs due to:
- Sudden upward hyperextension of the big toe during sports
- Repetitive bending and stress on the metatarsophalangeal (MTP) joint
- Insufficiently supportive footwear
- Hard playing surfaces like turf fields
Sports such as football, soccer, martial arts, dance, and basketball are commonly associated with this condition.
Symptoms of Turf Toe
- Pain, tenderness, or swelling at the base of the big toe
- Reduced range of motion or stiffness
- Difficulty pushing off the foot while walking or running
- A “popping” sensation during the injury in acute cases
Symptoms can range from mild to severe, depending on the extent of ligament damage.
Diagnosing Turf Toe
We use the following tools to ensure an accurate diagnosis:
- Hands-on joint examination for swelling, laxity, and pain
- Weight-bearing X-rays to rule out fractures
- Ultrasound or MRI if soft tissue damage or joint instability is suspected
Treatment for Turf Toe
Non-Surgical Treatment
- RICE therapy (Rest, Ice, Compression, Elevation)
- Immobilization with a walking boot or stiff-soled shoe
- Taping or toe strapping to limit motion
- Custom orthotics with a Morton’s extension to stabilize the toe joint
- Shockwave therapy or MLS laser therapy to accelerate healing
- Physical therapy to restore strength and range of motion
Surgical Treatment
In cases of severe ligament tear, chronic instability, or failed conservative care, surgical repair may be necessary. Our team provides referrals to trusted foot and ankle surgeons and coordinates post-operative care and rehabilitation.
What to Expect from Recovery
Mild cases may heal within a few weeks, while more moderate to severe turf toe injuries may require several months for full recovery. Early treatment significantly improves outcomes.
