 Toenail Trauma

What Is Toenail Trauma?

Toenail trauma refers to injury or damage to the toenail and its surrounding tissues. It can result from direct impact, repetitive pressure, or chronic irritation. At SoCal Foot and Ankle Doctors, conveniently located in Santa Monica and the Cedars-Sinai Medical Towers, we treat both acute and chronic toenail trauma with precision to restore nail health and prevent complications.

Common Causes of Toenail Trauma

  • Dropping heavy objects on the toe
  • Stubbing the toe
  • Repetitive pressure from tight shoes or long-distance running
  • Sports injuries
  • Nail salon or grooming-related trauma
  • Improper nail trimming

Symptoms of Toenail Injury

  • Pain and tenderness
  • Swelling around the nail
  • Bruising or blood under the nail (subungual hematoma)
  • Nail thickening or deformity
  • Nail detachment or lifting
  • Discoloration (black, purple, yellow)

Risks of Untreated Toenail Trauma

  • Chronic pain and deformity
  • Nail fungus (onychomycosis) secondary to lifting or cracks
  • Ingrown toenails
  • Infection of the nail bed or surrounding skin
  • Permanent nail loss or scarring

Diagnosis

Our podiatrists will:

  • Evaluate for fungal infection or subungual masses if the nail appears thick or discolored
  • Examine the nail and surrounding skin
  • Use digital X-rays to check for fractures of the toe bone (distal phalanx)

Treatment Options

Acute Toenail Injuries

  • Drainage of subungual hematoma to relieve pain
  • Splinting or offloading injured toes
  • Antibiotics if infection is present
  • Partial or full nail removal if the nail is severely damaged

Chronic or Recurrent Trauma

  • Custom shoe modifications or orthotics to offload pressure
  • Cosmetic nail restoration for thickened or damaged nails
  • CN-U Nail Solution to restore nail clarity and health
  • Laser treatment if fungal infection is suspected

Surgical Options

  • Matrixectomy (permanent removal of part of the nail) in recurrent ingrown toenail cases

Home Care Tips

  • Wear protective, properly-fitted footwear
  • Keep toenails trimmed straight across
  • Avoid trauma-inducing activities until fully healed
  • Use topical antifungals if fungal infection is suspected
